Surgical Treatment of Extravasation Injuries

The authors present their experience of treating anti-cancer drug extravasation by means of a composite surgical technique that consists of infiltration with physiological solution and hyaluronidase and subsequent manual aspiration of solutes alternated with profuse irrigation of the infiltrated area. In the immediate post-op we carry out a medical therapy that consists of calciparine and topic antibiotic and/or steroid creams. Since the year 2000 this technique has been used on 25 patients. Management of chemotherapy extravasation: ESMO–EONS Clinical Practice Guidelines

Extravasation is the process by which any liquid (fluid or drug) accidentally leaks into the surrounding tissue. In terms of cancer therapy, extravasation refers to the inadvertent infiltration of chemotherapy into the subcutaneous or subdermal tissues surrounding the intravenous or intra-arterial administration site. Extravasated drugs are classified according to their potential for causing damage as ‘vesicant’, ‘irritant’ and ‘nonvesicant’ (Table 1). Some vesicant drugs are further classified into two groups: DNA binding and non-DNA binding. Allwood et al. (2002) divided the drugs into vesicants, exfoliants, irritants, inflammitants and neutrals.

Photodynamic therapy is under intense investigation as a possible adjuvant for the treatment of malignant tumors of the central nervous system. It relies on the fact that photosensitizers are selectively taken up or retained by malignant tissue. However, most brain tumors are accompanied by substantial vasogenic edema as a consequence of blood-brain barrier disruption within the tumor, leading to extravasation and propagation of plasma constituents into the surrounding brain tissue. Effectiveness of oral N -acetylcysteine in a rat experimental model of asthma

Oxidative stress appears to be relevant to asthma pathogenesis. Therefore, the effectiveness of the antioxidant N -acetylcysteine was examined on antigen-induced pulmonary responses in sensitized Brown-Norway rats. Blood-brain barrier disruption by low-frequency ultrasound.

Insights in ChAdOx1 nCoV-19 vaccine-induced immune thrombotic thrombocytopenia

Abstract SARS-CoV-2 vaccine ChAdOx1 nCoV-19 (AstraZeneca) causes a thromboembolic complication termed vaccine-induced immune thrombotic thrombocytopenia (VITT). Using biophysical techniques, mouse models, and analysis of VITT patient samples, we identified determinants of this vaccine-induced adverse reaction. Super-resolution microscopy visualized vaccine components forming antigenic complexes with platelet factor 4 (PF4) on platelet surfaces to which anti-PF4 antibodies obtained from VITT patients bound. Laparoscopic Treatment of Endoscopic Sigmoid Colon Perforation

Perforation of the colon after colonoscopy is a rare but potentially lethal complication. It usually occurs when endoscopy is performed for therapeutic purposes. In these cases the election of the best treatment is difficult and still controversial. Laparoscopy is a new approach for diagnosis and treatment of this condition. We report a case of a patient who was initially treated laparoscopically after a colonoscopic perforation. A postoperative leak was detected and the patient underwent open surgery. Possible therapeutic approaches and a literature review are discussed.

Radiotherapy (RT) plays a key role in cancer treatment. Although the benefit of ionizing radiation (IR) is well established, some findings raise the possibility that irradiation of the primary tumor not only triggers a killing response but also increases the metastatic potential of surviving tumor cells. Here we addressed the question of whether irradiation of normal cells outside of the primary tumor augments metastasis by stimulating the extravasation of circulating tumor cells.
